We are seeking a highly motivated and experienced Senior Community Nurse to join our Adult Community Nursing Service team in Hounslow. The successful candidate will have a strong background in community nursing, with experience in assessing, planning, delivering, and evaluating patient care in their own homes. You will also have excellent leadership and management skills, with the ability to work to a high performance level and contribute to the development of a skilled, competent, and motivated workforce.
Key Responsibilities- Manage a designated caseload of patients, ensuring holistic assessments are undertaken by a competent practitioner within the nursing team.
- Assist the Community Team Manager in the management of the team, ensuring effective leadership and management of the caseload.
- Develop and implement care plans that meet the needs of patients, working closely with other healthcare professionals to ensure seamless care.
- Provide supervision and support to junior members of the team, promoting a culture of learning and development.
- Contribute to the development of the team, identifying areas for improvement and implementing changes to improve patient outcomes.
- Qualified RGN (Registered General Nurse - Adult) with current registration with NMC - Part 1.
- Diploma or BSc (Hons) Community Health (DN).
- Experience in assessing, planning, delivering, and evaluating patient care in their own homes.
- Excellent leadership and management skills, with the ability to work to a high performance level.
- Ability to work collaboratively with other healthcare professionals to ensure seamless care.
