Job Title: Adult Community Nurse

We are seeking a highly skilled and compassionate Adult Community Nurse to join our team at Bank Partners. As a key member of our community healthcare team, you will be responsible for providing high-quality nursing care to housebound patients with complex needs, enabling them to remain in their own homes.

Key Responsibilities:
  • Provide skilled nursing care to patients with complex needs, including assessments, planning, implementation, and evaluation of individual care programs.
  • Work collaboratively with specialist nurses, allied health, and social care professionals to meet patient needs and prevent unnecessary A&E attendance or hospital admissions.
  • Conduct first assessments and reviews, and develop and implement care plans that meet the unique needs of each patient.
  • Be flexible and adaptable to support a corporate approach to patient care, working across the Hounslow and Richmond areas.
Requirements:
  • Registered General Nurse with a strong commitment to delivering high-quality patient care.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team.
  • Full UK driving license with access to a car, with business mileage allowance paid per mile.
