Job Overview

We are seeking a highly skilled and compassionate Registered General Nurse to join our Adult Community Day Nursing Service at Hounslow and Richmond Community Healthcare (HRCH). As a key member of our team, you will provide high-quality nursing care to housebound patients with complex needs, enabling them to remain in their own homes.

Main Responsibilities
  • Provide skilled nursing care to patients with complex needs, promoting independence and well-being.
  • Work collaboratively with the Community Day Nursing Team Manager to plan, implement, and evaluate individual care programs.
  • Conduct first assessments and reviews, monitoring patient progress and adjusting care plans as needed.
  • Collaborate with specialist nurses, allied health, and social care professionals to meet patient needs and prevent unnecessary hospital admissions.
  • Be flexible to support a corporate approach to patient care, working across the Hounslow and Richmond areas.
Requirements
  • Registered General Nurse with a commitment to delivering high-quality patient care.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team.
  • Full UK driving license with access to a car (business mileage allowance paid per mile).
