About the Role:

We are seeking a highly skilled and dedicated Support Service Coordinator to join our Care and Support Team at Thirteen Group. As a key member of our team, you will be responsible for providing high-quality support services to our customers, including young parents and their families, in our supported housing scheme in Durham.

Key Responsibilities:
  • Develop and implement person-centred support plans to meet the needs of our customers.
  • Provide regular keywork sessions to review progress and set new goals.
  • Manage risk and safeguarding for all residents, ensuring their safety and well-being.
  • Work collaboratively with other teams to provide a comprehensive support service.
  • Assist customers to access benefits and grants, and provide guidance on financial management.
  • Contribute to the development of our support services, identifying areas for improvement and implementing changes.
About You:

We are looking for an experienced and compassionate individual who is passionate about supporting vulnerable client groups. You will have a good understanding of adult and child safeguarding, and be able to work independently and as part of a team.

You will be able to prioritize your workload, manage competing demands, and maintain accurate records. A driving licence and access to your own vehicle are essential for this role, as you will be required to work on an outreach basis across County Durham.
