Position Overview
The Patient Care Pathway Manager plays a pivotal role in collaborating with healthcare professionals and Service Managers to ensure the seamless management of patient care pathways.
This position is responsible for overseeing the patient journey from initial referral through to the first outpatient consultation, diagnostics, treatment planning, execution, and follow-up. The role includes identifying potential delays in the patient pathway and ensuring precise data collection to facilitate the process.
The work schedule involves rotating shifts between 8am to 4pm and 10am to 6pm.
Key Responsibilities
Maintain a comprehensive understanding of 18-week and cancer waiting time targets, adhering to relevant guidelines and proactively ensuring compliance with all targets. Provide weekly updates to Service Managers regarding any delays in patient pathways or potential breaches of waiting time targets. Attend performance review meetings regularly, ensuring that agreed actions and outcomes are documented and monitored. Manage the scheduling and distribution of outpatient appointment letters and communicate with patients as necessary. Collaborate with consultants and their teams to ensure accurate case mix on surgical lists, notifying relevant staff of any changes in accordance with established protocols. Ensure that patient cancellations are promptly rescheduled within specified timeframes. Oversee pre-assessment clinics, working closely with pre-assessment staff to ensure efficient clinic operations. Monitor elective surgery and imaging waiting times, proactively identifying risks and implementing corrective measures to prevent breaches of relevant targets in conjunction with service and clinical teams.Quality Assurance
Utilize Trust IT systems to monitor the quality of patient pathways and take action to enhance the quality of information within the Directorate.Validate and analyze data, producing ad hoc reports as required by the Service Manager.
Use hospital information systems to document and review both clinical and non-clinical patient information. Assist Service Managers in the collection and analysis of pertinent performance metrics. Ensure the transfer of patient information aligns with Trust policies and procedures regarding patient records and clinic operations.Candidate Profile
Excellent communication and interpersonal skills. Strong organizational abilities. Capability to work effectively within a team and independently. Previous experience in managing waiting lists and supervisory roles is essential.Knowledge and Experience Requirements
A solid educational background, with a minimum of grade C in English language and mathematics at GCSE level or equivalent. Experience working within the NHS or a similarly sized organization. Exceptional administrative and organizational skills, with prior experience in managing waiting lists and supervisory responsibilities.Analytical Skills
Ability to function within a multidisciplinary team and prioritize an unpredictable workload. Proficiency in IT, including knowledge of databases and report generation using Excel spreadsheets.
