Job Title: Discharge Coordinator

Job Summary:

Livewell Southwest is seeking a highly skilled and compassionate Discharge Coordinator to join our team. As a key member of our Mental Health Service, you will play a vital role in ensuring the smooth transition of patients from our in-patient services to community-based care.

Key Responsibilities:
  • Develop and implement discharge plans that meet the individual needs of patients, taking into account their social, housing, and financial circumstances.
  • Work closely with multidisciplinary teams, including healthcare professionals, social workers, and community agencies, to ensure seamless communication and coordination.
  • Identify and address barriers to discharge, such as housing and financial issues, and provide support to patients to overcome these challenges.
  • Monitor patient progress and bed availability, ensuring timely and effective discharge planning.
  • Collaborate with the Discharge Team Leader and other stakeholders to ensure that discharge plans are aligned with organizational goals and objectives.
Requirements:
  • NVQ Level 5 or equivalent qualification in a relevant field, such as mental health or social care.
  • Proven experience of working in a mental health setting, with a focus on discharge planning and coordination.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a flexible approach to working hours and shifts.
  • Knowledge of local health and social care services, including community resources and support agencies.
