Position Overview

An exceptional opportunity is available for individuals with prior experience in the NHS and a keen interest in oncology services to assist in our Haematology pathway through a new patient-centric role.

The Oncology Patient Support Coordinator (OPSC) will be an integral member of the Clinical Nurse Specialists (CNS) team, collaborating closely with nursing staff to serve as a primary contact for patients within the Haematology specialty. This role will work in conjunction with Multi-Disciplinary Team (MDT) Facilitators, Diagnostic Services, and administrative teams to ensure smooth coordination and timely management of patients throughout their care journey.

This position is directly patient-facing, acting as a consistent point of contact for routine, suspected, and confirmed cancer patients and their families throughout the entire cancer care continuum.

This role also involves contributing to the National Living with and Beyond Cancer initiative.

At the time of the interview, candidates will be required to complete a task and an exercise.

Key Responsibilities

The primary aim of this role is to enhance and support the existing specialty cancer team in guiding patients from referral through diagnosis and into treatment and beyond, by delivering a safe, seamless, and patient-centered service. This includes:

  1. Maximizing the health and quality of life for patients and their caregivers while improving the quality and efficiency of healthcare delivery.
  2. Being accountable for practice and behavior under the guidance of a registered practitioner while working as part of the specialty cancer multidisciplinary team.
  3. Collaborating with primary care professionals to understand cancer pathways, ensuring newly referred patients attend appointments and investigations, and promoting timely communication.
  4. Coordinating high-quality patient care through ongoing assessments and proactive identification of needs using established tools and procedures.
  5. Serving as the primary contact for all patients under the specialty cancer team's care, including those on personalized follow-up, managing patient inquiries, and facilitating rapid re-entry into the system as needed.
  6. Working with the clinical team to organize necessary assessments, appointments, and investigations to expedite patient care.
  7. Navigating the complex health and social care landscape to support patients at every stage of their cancer journey, building relationships with professionals and facilitating access to various services.
  8. Conducting Holistic Needs Assessments and coordinating care for patients with non-complex needs to ensure timely access to appropriate support.
  9. Recognizing potential deterioration in patient conditions and escalating concerns to a registered practitioner promptly.
  10. Engaging in regular supportive conversations with patients, assessing their holistic needs, and providing relevant information and advice while connecting them to additional resources.
  11. Maintaining awareness of Cancer Waiting Time Targets and ensuring accurate and timely documentation of all patient care coordination activities.

Reporting any incidents through the appropriate channels to foster an open and transparent culture is crucial.

Clinical Responsibilities

Responsibilities include triaging incoming calls, highlighting the need for practical and emotional support, and making proactive outreach calls to assess patient needs.

Maintaining databases for monitoring patient follow-up and coordinating care plans for patients with non-complex needs is also required.

Supporting the delivery of patient information and ensuring all patients and caregivers receive timely communications regarding their care.

Participating in the development and evaluation of health and wellbeing sessions in collaboration with relevant program leads.

Qualifications and Experience

Essential:

  • GCSE English Language and Mathematics (Grades A-C) or equivalent.
  • Relevant NVQ Level 3 or equivalent training in administration or healthcare.
  • Recognized ECDL or equivalent proficiency in Microsoft Office.

Desirable:

  • Diploma in administrative practice or business management.
  • Foundation degree in health and social care or equivalent higher apprenticeship.

Skills and Personal Qualities

Essential skills include flexibility in prioritizing work, effective communication, and the ability to handle confidential information responsibly. Candidates should demonstrate strong time management skills and the ability to cope with potentially stressful situations.

Willingness to work across various Trust sites is essential, and having independent means of transport is desirable.
