We are seeking a dedicated and compassionate Care and Support Worker to join our Reablement Service team. As a Care and Support Worker, you will play a vital role in providing high-quality care and support to individuals in their own homes, helping them to regain independence and confidence.
**Key Responsibilities**
* Provide person-centered care and support to individuals with disabilities and those recovering from illness or injury
* Assist with daily living tasks, such as personal care, meal preparation, and household chores
* Support individuals to relearn skills and maintain their independence
* Work collaboratively with other healthcare professionals to ensure seamless care delivery
