About the Role

We are seeking an experienced Advanced Clinical Practitioner to join our team at Derbyshire Healthcare NHS Foundation Trust. As a key member of our multidisciplinary team, you will provide high-quality, patient-centered care to individuals with complex physical health needs.

Key Responsibilities
  • Provide autonomous care to patients within the older adult inpatient services, working closely with the multidisciplinary team to share leadership and specialist knowledge.
  • Undertake physical and mental health examinations, analyzing complex clinical signs and investigation results to consider differential diagnoses, formulate decisions, and diagnose conditions.
  • Initiate treatment as required, deciding when necessary to refer to senior medical colleagues.
  • Provide clinical leadership to the ward team and be a valued member of the medical team.
  • Support the establishment and delivery of high-quality, evidence-based care, working across all clinical areas to the standards established within the Derbyshire Advanced Practice Network.
Requirements
  • Qualified/registered Advanced Clinical Practitioner.
  • Experience of working in a team and leading a team.
  • Evidence of portfolio.
