About the Role
We are seeking a highly skilled and dedicated Social Worker to join our Adult Central South Neighbourhood team at Bolton Council. As a key member of our team, you will play a vital role in providing support to adults and their carers who may be eligible for assessment and care services under the Care Act 2014.
Key Responsibilities
- Conduct comprehensive assessments to identify the needs of adults and their carers, and develop personalized care plans to ensure they receive the necessary support.
- Work closely with health colleagues, other professionals, and community groups to provide a holistic approach to care and support.
- Undertake ongoing reviews and evaluations to ensure the effectiveness of care plans and make adjustments as needed.
- Participate in Best Interests meetings and contribute to the development of care and support plans that prioritize the individual's needs and preferences.
- Collaborate with other teams and agencies to ensure a seamless delivery of care and support services.
- Develop and maintain strong relationships with service users, their families, and carers to ensure they feel supported and empowered.
Requirements
- Hold a degree in social work and be registered with Social Work England.
- Have a strong understanding of the Care Act, Mental Capacity Act, and Adults Safeguarding.
- Be able to work effectively in a fast-paced environment, prioritizing tasks and managing competing demands.
- Demonstrate a commitment to person-centered practice and a strength-based approach to social work.
- Be able to communicate effectively with service users, colleagues, and other professionals.
