Detailed Job Description and Main Responsibilities

The successful candidate will be responsible for providing and promoting the highest possible standards of care, based on professionally determined standards. They will accept accountability for the patient as an individual, providing autonomy and encouraging teamwork through effective leadership and being a role model.

The post holder will assess care and provide a seamless service from receiving the referral through to the completion of care. They will also be responsible for developing and maintaining a continuing and caring relationship with regular patients and their carers, ensuring patient confidentiality, dignity, and autonomy are upheld.

The successful candidate will be expected to:

  • Assess, plan, implement, and evaluate care and interventions for patients with complex and multifactorial needs.
  • Act as a trouble shooter, resource, mentor, and clinical supervisor for colleagues as appropriate.
  • Support the senior sister to develop and maintain safe systems of work, including policy development and risk assessments.
  • Assist in the investigation of incidents and complaints, and ensure appropriate documentation is completed.
  • Develop and maintain a continuing and caring relationship with regular patients and their carers, ensuring patient confidentiality, dignity, and autonomy are upheld.
