Position Overview

We are pleased to present a rewarding opportunity for a Rehabilitation Therapist within our dedicated team. Under the guidance of a senior Occupational Therapist, you will collaborate with a diverse group of professionals in short-term service delivery, focusing on enhancing the independence of our clients.

Key Responsibilities

  • Conduct comprehensive evaluations and provide the interdisciplinary team and families with insights regarding suitable levels of assistance, adaptive tools, and minor modifications.
  • After a thorough assessment in the client's residence, offer tailored recommendations to support their ability to remain at home, including necessary assistance, adaptive devices, and minor adjustments.
  • Perform specialized evaluations, including cognitive, perceptual, wheelchair, caregiver assessments, and Multi-Factorial Falls Assessments.
  • Educate clients on safely performing daily tasks; provide guidance on specialized techniques such as energy conservation, joint protection, and task simplification.
  • Support caregivers with advice on care strategies and health promotion to sustain community-based care for as long as feasible.

Communication and Collaboration

Maintain effective communication channels among:

  • Multi-disciplinary teams to ensure cohesive and well-coordinated care for clients.
  • Family members and caregivers to discuss the client's condition and related concerns while respecting confidentiality.
  • Relevant social services and agencies involved in the client's care, adhering to confidentiality and data protection regulations.

Communicate sensitive information regarding clients' health and social welfare, particularly with those experiencing sensory or cognitive challenges.

Analytical Skills and Decision Making

Assess the effectiveness of care provided and adjust the occupational therapy approach as needed to ensure optimal treatment outcomes, utilizing appropriate evaluation measures.

Planning and Organization

In alignment with professional standards, evaluate client needs and collaborate to design and implement an Occupational Therapy program that ensures the highest quality of care.

After conducting a holistic assessment, provide the client, interdisciplinary team, and family with recommendations for discharge, including necessary support, adaptive equipment, and modifications.

Manage your own caseload effectively.

Physical Competence

Conduct manual handling risk assessments; advise on suitable handling techniques; assess and arrange for the provision of necessary manual handling equipment.

Client Care

Perform holistic assessments and provide the interdisciplinary team and family with recommendations regarding discharge from care facilities, including appropriate support, adaptive tools, and minor modifications.

After a comprehensive assessment in the client's home, deliver tailored recommendations to help maintain their independence, including necessary support, adaptive devices, and minor adjustments.

Policy and Service Development

In accordance with professional standards, assess, plan, and implement the Occupational Therapy program for each client to ensure the highest quality of care.

Adhere to health and safety policies to maintain a safe environment for clients, visitors, and staff.

Resource Management

Ensure that social care services and equipment are provided in compliance with relevant criteria.

People Management and Training

Promote understanding of the Occupational Therapist's role within your area of work and assist in training Occupational Therapy Technical Instructors/Assistants, students, and other relevant personnel to facilitate effective interdisciplinary collaboration.

Provide clinical support and supervision to support staff.

Mentor or supervise students during clinical placements.

Participate in mandatory in-service training.

Teach support staff techniques for client care, including the use of adaptive equipment and manual handling methods.

Information Technology and Administrative Duties

Engage in clinical audits as required for governance purposes.

Comply with documentation and reporting standards to demonstrate the provision of an efficient and effective quality service.

Research and Development

Ensure continuous professional development to maintain high standards of client care; maintain an up-to-date CPD portfolio.

Participate in supervision and annual review processes to achieve personal development and uphold high standards of client care.

Qualifications and Experience

Essential

  • Degree in Occupational Therapy
  • Ability to apply clinical reasoning
  • Goal-setting skills

Desirable

  • Experience in a reablement setting
