Practice Nurse

We are seeking an experienced and skilled Practice Nurse to join our team at East Staffordshire Primary Care Network. As a Practice Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.
Key Responsibilities- Assess patients' health needs and develop, implement, and evaluate individualized care plans to meet their specific needs.
- Provide comprehensive nursing care, including health promotion, disease prevention, and management of chronic conditions.
- Work collaboratively with other healthcare professionals to ensure seamless patient care and achieve optimal health outcomes.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and safety.
- Maintain accurate and up-to-date patient records, ensuring compliance with regulatory requirements and organizational standards.
- Stay current with evidence-based practice and participate in ongoing professional development to enhance clinical skills and knowledge.
- Registered Nurse with the Nursing and Midwifery Council (NMC)
- Minimum 2 years of post-registration experience in a community or primary care setting
- Strong clinical skills and experience in treatment room consultations
- Ability to assess complex health needs and develop, monitor, and review care plans
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
- Strong problem-solving and critical thinking skills, with the ability to prioritize tasks and manage time effectively
- Ability to maintain confidentiality and handle sensitive information with discretion
