Position Overview
The role is designed to assist the Practice Based Clinical Pharmacist in delivering safe, effective, and efficient systems for managing repeat prescriptions, optimizing medication use, minimizing waste, and enhancing patient outcomes.
Key Responsibilities
- Conduct thorough medication reconciliation in line with established practice protocols, ensuring accurate updates to patient medical records and prescriptions.
- Aid in achieving the practice's prescribing Quality and Outcomes Framework objectives.
- Collaborate with the Practice Pharmacist to review and refine repeat prescribing protocols and Standard Operating Procedures, enhancing the efficiency and effectiveness of the prescribing process.
- Provide informed guidance to healthcare professionals and patients regarding changes in prescribing to improve safety, quality, and cost-effectiveness.
- Participate in the monitoring and management of prescribing budgets, liaising with the medicines management team as necessary.
- Develop evidence-based resources and information to facilitate the implementation of rational and cost-effective prescribing practices.
- Support the Practice Based Clinical Pharmacist in conducting medication reviews, creating patient information materials, and raising awareness about medications, including community outreach as appropriate.
- Proactively manage the safe handling of prescriptions, assessing risks and recommending improvements.
- Address medication-related inquiries from patients and staff professionally, referring to the appropriate healthcare provider as per practice protocols.
- Advise on the sourcing and safe management of medications as required.
- Assist in the submission of claims for personally administered medications as needed.
About Our Organization
Barton Family Practice is comprised of a dedicated team including GP Partners, a Salaried GP, a trainee Advanced Nurse Practitioner, a Clinical Pharmacist, Chronic Disease Nurses, a Practice Nurse, a Health Care Assistant, and a Phlebotomist. Our administrative team includes reception and administrative staff, along with a Practice Manager.
We operate as a non-dispensing practice, serving a patient population of 7,200, located in a semi-rural area with convenient access to various regional attractions.
Employment Type
This is a part-time position, requiring 20-30 hours per week, with a schedule from Monday to Friday.
