We are seeking a dedicated and qualified Medication Management Specialist to join our dynamic Primary Care Team at Carlton Group Practice. This full-time position is ideal for a professional with a strong background in pharmaceutical science and current registration, looking to make a significant impact in patient care.
Key Responsibilities
In this role, you will collaborate closely with General Practitioners and Clinical Pharmacists to facilitate optimal medication use. Your primary responsibilities will include:
- Engaging with patients to ensure effective medication management through shared decision-making.
- Conducting medicine optimisation tasks, including verifying inhaler techniques and supporting medication reviews.
- Processing prescription inquiries and reconciling medications to enhance patient safety.
- Assisting in medication reviews for new care home patients and synchronising medications during patient transfers.
- Providing expert advice on public health and social care needs, including lifestyle modifications and service information.
- Participating in initiatives aimed at promoting responsible antibiotic use and reducing inappropriate prescribing.
- Supporting the implementation of national prescribing guidelines and conducting clinical audits.
- Training administrative staff on the safe handling of medications and the Electronic Prescription Service (EPS).
- Collaborating with the clinical team to resolve medication-related queries and ensure timely medication reconciliation.

Qualifications
Essential
- Current registration with the General Pharmaceutical Council (GPhC).
- BTEC/NVQ level 3 or equivalent in pharmaceutical science/pharmacy services.
- Evidence of ongoing professional development.
- Strong educational background in English, Science, and Maths.
Experience
Essential
- A minimum of 2 years of post-qualification experience.
- Solid understanding of clinical principles and evidence-based healthcare.
- Experience managing a variety of clinical conditions.
- Background in primary care, community, or hospital settings.
Desirable
- Familiarity with systems that support patient management in primary care.
- Experience with SystmOne.
- Comprehensive clinical pharmacy knowledge, including relevant terminology.
