Job Overview

Pennine Care NHS Foundation Trust is seeking a highly skilled and experienced medical professional to lead our Community Rehabilitation Team (CRT). The successful candidate will be responsible for providing senior clinical leadership to the CRT service and ensuring the medical care of patients under the team's care.

The CRT is a fully integrated team that aims to provide intensive in-reach work while patients are on the ward, building trusting relationships with service users prior to discharge and ensuring continuity of care when they leave the ward. The team's focus is on social, educational, and occupational recovery, with the aim of maximum symptomatic and functional recovery and the prevention of relapses.

The ideal candidate will have excellent clinical skills, using a bio-psycho-social perspective and wide medical knowledge. They will also have experience in rehabilitation or forensic psychiatry, as well as experience of doing audit and a commitment to participating in the Trust's audit programme. Additionally, they will have extensive experience of assessing and managing clinical risk.

The successful candidate will be responsible for:

  • Providing senior clinical leadership to the CRT service
  • Ensuring the medical care of patients under the team's care
  • Leading weekly MDT care planning/risk management planning meetings
  • Responsible Clinician responsibilities with attendance at and report-writing for Mental Health Tribunals and Managers hearings
  • Compliance with all legal obligations covering clinical practice
  • Actively managing the team's caseloads and responding to new patients as they are referred

The team is expected to work closely with other departments within the Trust, including the Low Secure Units and the Rehabilitation and High Support Care Hub. The successful candidate will be required to travel to various locations within the Trust and will need to have a valid UK driving licence.
