Job Title: Adult Clinical Lead

Sonderwell is seeking a highly skilled and experienced Adult Clinical Lead to join our team. As a key member of our leadership team, you will be responsible for leading the delivery of complex care into the home, ensuring the highest standards of quality and patient-focused care.

Key Responsibilities:
  • Develop and implement individualized care plans in partnership with service users, families, and healthcare professionals.
  • Conduct initial holistic nursing assessments, involving other statutory, voluntary, and third-party sector providers as necessary.
  • Motivate and manage a team of nurses and care support workers, embedding our company values and behaviors.
  • Manage and support field-based care support workers and nurses through regular supervisions, competency assessments, and appraisals.
  • Maintain a strong commitment to quality standards, ensuring compliance with statutory, mandatory, and contractual requirements.
  • Collaborate with Central Support to motivate carers and nurses through informal check-ins, formal supervision, and appraisals.
  • Oversee the development and implementation of rotas, ensuring field staff are appropriately trained, assessed, and signed off as competent prior to lone working.
Requirements:
  • Professional nursing registration with no restrictions.
  • Experience of working in the community, including Spinal Injury, Acquired Brain Injuries, and Cerebral Palsy.
  • Ability to develop effective working relationships with key stakeholders, including Case Managers, Integrated Care Systems, General Practitioners, Consultants, and other Healthcare Professionals.
  • Strong commitment to quality standards and flexibility to respond to shifting priorities.
  • Full Driving Licence.
