Position Overview

We are seeking dedicated Community Support Specialists to join our team at HCRG Staffing. This role focuses on providing essential support to adults in need, ensuring their well-being and access to necessary resources.

Key Responsibilities

The successful candidates will be responsible for:

  • Conducting Comprehensive Assessments: Carry out thorough evaluations to understand the needs of individuals.
  • Developing Support Plans: Collaborate with clients to create tailored support strategies.
  • Facilitating Access to Services: Guide clients through the process of obtaining care and support.
  • Emphasizing Reablement: Focus on enhancing independence through assistive technology and community resources.
Work Environment

This position offers a hybrid working model, allowing for flexibility with three days in the community and two days remote.
