We are seeking an experienced and skilled Lead Nurse to join our team at NHS West Yorkshire Integrated Care Board. The successful candidate will be responsible for leading the Continuing Healthcare (CHC) team and ensuring the delivery of high-quality care to patients across West Yorkshire.
Key Responsibilities- Lead the CHC team in the assessment, care planning, and evaluation of packages for patients eligible for NHS Continuing Healthcare.
- Support the investigation of complaints and review of NHS CHC retrospective cases, appeals, and independent panel review process.
- Develop and maintain effective communication networks with health and social care professionals to ensure a seamless and efficient service delivery.
- Exercise professional judgment in analyzing and communicating complex and sensitive information on behalf of the CHC service.
- Support Community Matrons/District Nurses in their case management roles for complex cases that are eligible for continuing healthcare.
- Registered Nurse (RGN/RMN/RNLD/RSCN) with current and relevant NMC registration.
- Post Graduate qualification or specialist training in Community Nursing.
- Significant experience of both community and hospital working and a sound understanding of the NHS CHC national guidance.
- Excellent communication and interpersonal skills, with the ability to work effectively with colleagues, patients, and families.
- Ability to exercise independent judgment and problem-solving skills.
