We are seeking a highly skilled and experienced Mental Health Practitioner to join our Bucks Gateway team. As a key member of our multidisciplinary team, you will be responsible for providing comprehensive telephone assessments for individuals in need of mental health support.
Key Responsibilities- Conduct thorough telephone assessments to identify individual needs and develop personalized care plans.
- Collaborate with healthcare professionals and community agencies to ensure seamless care coordination.
- Provide expert advice and guidance to referrers and service users on mental health matters.
- Work closely with the Gateway team to ensure timely and effective delivery of mental health services.
- Contribute to the development and implementation of service improvement initiatives.
- Registered qualification in nursing, social work, or occupational therapy.
- Significant post-qualification experience in mental health settings, working with adults with mental illness.
- Experience of mentoring and clinical supervision of others.
- Ability to manage a caseload and act as a care coordinator for clients.
- Excellent communication and interpersonal skills.
