We are seeking a compassionate and dedicated Support Worker to join our Health and Care service. As a Support Worker, you will play a vital role in providing practical and emotional support to individuals in need.
Key Responsibilities- Providing reactive, focused, and flexible support to adults following a hospital stay to enable early discharge.
- Responding to referrals from Clinicians and Community Health Professionals
- Assessing service users' needs, completing and following a support plan for the individual.
- Supporting service users in the weeks after hospital discharge or to prevent readmission.
- Taking someone to a hospital appointment that they otherwise couldn't get to or be picking up prescriptions and shopping.
- Good knowledge of services provided by the NHS and Social Care.
- Ability to make things great, improving service quality for the benefit of users.
- Professionalism, dealing with queries in a diplomatic and confidential manner.
- Flexibility, working hours out with the norm suits you.
