Job Title: Community Health Nurse

We are seeking a skilled and dedicated Community Health Nurse to join our team at County Durham and Darlington NHS Foundation Trust. As a Community Health Nurse, you will play a vital role in delivering high-quality patient care and supporting our community respiratory nursing service.

About the Role

This is an exciting opportunity to work in a dynamic and supportive environment, where you will have the chance to develop your skills and expertise in respiratory nursing. You will be responsible for managing a caseload of complex respiratory patients, working closely with our specialist respiratory nursing team to provide holistic care and support.

Key Responsibilities
  • Assess and manage the health needs of patients with respiratory conditions, including COPD and other chronic respiratory diseases.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Work collaboratively with other healthcare professionals to provide comprehensive care and support.
  • Conduct home visits and provide education and support to patients and their families.
  • Participate in clinical supervision and peer review to ensure high-quality care and continuous learning.
Requirements
  • Nursing Midwifery Council (NMC) registration.
  • Experience in community nursing, preferably in a respiratory care setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to travel independently and work flexibly, including evenings and weekends.
