Job Title: Community Health Nurse

We are seeking a skilled and dedicated Community Health Nurse to join our team at County Durham and Darlington NHS Foundation Trust. About the Role
  • Deliver respiratory care in out-patient clinics and in patients' homes, supporting the specialist respiratory nursing service.
  • Contribute to creating improvement, transformation, and innovation within the service to promote best practice.
  • Commit to continuous personal development and lifelong learning by attending mandatory training and adhering to all trust policies and procedures.
Key Responsibilities
  • To co-manage a caseload of complex respiratory patients, under the direction of the community respiratory nursing team, across the community respiratory specialist service.
  • To undertake holistic patient respiratory assessment and review, including social and psychological well-being, and progress treatment interventions and adapt interventions to meet the changing needs of the patient.
  • Knowledge of national and regional policies influencing respiratory disease.
  • Experience of working in a community setting.
Requirements
  • Nursing and Midwifery Council part 1 and 2 Registration.
  • Able to travel independently around County Durham and Darlington.
  • Flexible attitude to work.
Desirable Qualifications
  • Cognitive Behavioural Therapy qualification.
  • Diagnostic spirometry performance qualification and registration.
  • Knowledge of supplementary oxygen assessment and blood gas performance.
  • Basic knowledge of national and regional policies influencing respiratory disease.
