About the Role

We are seeking a highly motivated and enthusiastic Clinical Assessor to join our Continuing Health Care team at NHS Lincolnshire Integrated Care Board.

The successful candidate will be responsible for leading and contributing to multi-disciplinary assessments, reviewing patients in receipt of Continuing Healthcare or Funded Nursing Care, and ensuring that patients with complex needs are referred to or discussed with the relevant Case Manager.

Key Responsibilities
  • Conduct comprehensive assessments using the Decision Support Tool in line with the National Framework and Standard Operating Procedures.
  • Review patients in receipt of Continuing Healthcare or Funded Nursing Care in accordance with the Framework.
  • Ensure that patients with complex needs are referred to or discussed with the relevant Case Manager for further assessment and planning of care needs.
  • Develop strong working relationships with partner agencies, including acute hospital trusts, local authorities, and the independent sector.
Requirements
  • Up-to-date NMC Registration.
  • Experience in a clinical setting, including inter-agency and multi-agency working.
  • Evidence of Continued Professional Development.
Desirable Qualifications
  • Knowledge around Continuing Health Care.
  • Clinical Knowledge Base to ensure Comprehensive Holistic assessment.
  • Experience of Managing risk and reporting.
  • Negotiation and Conflict Management Skills and the ability to influence in formal settings.
