About the Role

We are seeking a highly motivated and compassionate Support Worker to join our team at The Action Group. As a Support Worker, you will play a vital role in providing a caring, efficient, and effective 24/7 support service to individuals who use our services.

Main Responsibilities:

  • Practice the TAG Key Worker system, including organizing support plans and case reviews with relevant stakeholders.
  • Promote the physical and emotional well-being of individuals, including supporting self-care and risk assessments.
  • Develop meaningful relationships with individuals, helping them gain insight into their behavior and attitudes.
  • Encourage independence in daily living activities and provide support when necessary.
  • Promote hobbies, interests, and community participation, including organizing day trips and activities.
  • Assist with personal care as required.
  • Maximize communication effectiveness and promote self-advocacy.
  • Advocate on behalf of individuals, liaising with external organizations as needed.
  • Work with relatives and carers to ensure their involvement in the individual's life.
  • Stay up-to-date with local services, amenities, and organizations available to individuals.

Requirements:

  • Registration with the Scottish Social Services Council (SSSC) and PVG clearance.
  • Ability to work flexibly, including regular working outwith weekday office hours and sleepover duties.
  • Good communication and teamwork skills.
  • Ability to maintain confidentiality and respect individual rights and responsibilities.
