About the Role

Frontier Consulting is seeking a qualified Social Worker to join our team as a Mental Health Specialist Advisor. As a key member of our team, you will play a vital role in supporting young people aged 16-25 who have been in the care of Wandsworth Borough Council.

Key Responsibilities
  • Case Management: You will be responsible for managing a caseload of 15 young people with moderate to acute mental health issues, providing them with support and guidance to achieve their goals.
  • Pathway Planning: You will work with young people to develop robust and detailed Pathway Plans, which will include individual and group activities to help them achieve their goals.
  • Collaboration and Liaison: You will work closely with the Therapeutic Service, Transitions Social Worker, and Adult's Social Care Social Worker/Care Coordinators to ensure a comprehensive support network for young people.
  • Assessment and Reporting: You will conduct assessments, including risk assessments, and maintain accurate and up-to-date case records in compliance with Council procedures and quality standards.
  • Knowledge and Skills: You will have a strong understanding of key legislation, including the Leaving Care Act 2000 and Social Work Act 2017, and be able to apply this knowledge in your work with young people.
Requirements
  • Qualified Social Worker with experience of working with the Care Act 2014 and within Adult Mental Health Services.
  • Effective oral and written communication skills, including formal report writing.
  • Ability to handle challenges and own workload with a commitment to achieving positive outcomes.
  • Proven experience in carrying out assessments, including risk assessments, collecting, and analysing relevant information and following guidance.
