Main Responsibilities- Provide managerial and professional leadership to the nursing and nursing support team
- Work closely with the Business and Operational Managers to deliver, plan and future-proof services
- Lead the team to provide an exemplary clinical service while ensuring practice targets are maintained and achieved to optimal levels
- Accountable for the delivery and standards to ensure safe, effective, person-centered care
- Manage and review performance and absence management
- Act as the Infection Prevention Control (IPC) Lead and work to the IPC Policy
- Develop and implement operational plans and strategies for service delivery
- Develop, implement, and embed health promotion and wellbeing programmes
- Implement and evaluate individual treatment plans for chronic disease patients
- Identify, manage, and support patients at risk of developing long-term conditions
- Provide routine nursing care to patients as required
- Diagnose and manage acute and chronic conditions
- Prescribe medication for patients in accordance with PGDs and own scope of practice
- Review patient medications to enhance compliance
- Oversee the provision of wound care services to patients
- Co-ordinate the provision of travel medicine services
- Process pathology results as required
- Oversee the provision of chronic disease clinics
- Maintain accurate clinical records
- Ensure read codes and templates are used effectively
- Develop, implement, embed, and participate in well-person clinics
- Chaperone patients where necessary
- Assist GPs with minor surgery and contraception when required

- Support the team in dealing with clinical emergencies
- Recognise, assess, and refer patients presenting with mental health needs
- Implement vaccination and screening programmes for adults and children
- Liaise with external services/agencies to ensure patient support
- Delegate clinical responsibilities appropriately
- Support the clinical team with safeguarding matters
- Understand practice and local policies for substance abuse and addictive behaviour
- Deliver opportunistic health promotion
- Review clinical practices and respond to national policies and initiatives
- Co-ordinate the nursing team rota
- Facilitate placement of Student Nurses within the Practice
- Develop and review audit protocols and process
- Develop, review, and implement nursing protocols
- Participate in local initiatives to enhance service delivery and patient care
- Support and participate in shared learning within the practice
- Participate in the review of significant and near-miss events
- Lead the planning and implementation of changes within the nursing team
- Support the practice team with reviewing and implementing practice policies and protocols
- Experience of working within a primary care environment
- Experience in all areas of Chronic Disease Management, including Diabetes
