This position is within the South Neighbourhood Team, and you will play a crucial role in guiding adults towards independent living. Your responsibilities will include managing a caseload, conducting Care Act Assessments, creating Support Plans, performing Mental Capacity Assessments, and making Best Interest Decisions, as well as connecting individuals to necessary internal and external services to promote well-being.
