Key Responsibilities- Deliver comprehensive nursing care to patients with complex needs, including diabetic management, wound management, and palliative care.
- Work closely with patients, families, and healthcare professionals to develop and implement personalized care plans.
- Conduct regular assessments and reviews to ensure patients receive the best possible care and support.
- Collaborate with multidisciplinary teams to provide seamless care and support to patients.
- Participate in clinical governance and quality improvement initiatives to ensure high standards of care.
- Registered Nurse qualification and experience in a community setting.
- ENB 998 or equivalent qualification.
- Strong clinical skills, including wound management, catheterization, and medicines management.
- Excellent communication and interpersonal skills.
- Ability to work autonomously and as part of a team.
- IT literate and able to use electronic patient records.
