Position Overview

At Penketh Health Centre, our Medication Management Specialists are vital to the seamless operation of our healthcare services, ensuring that our patients receive optimal pharmaceutical care. The ideal candidate will be dedicated to enhancing the quality of patient treatment through effective medication management.

Key Responsibilities

  • Collaborate with clinical staff to provide essential technical support.
  • Assist the prescription team in addressing daily medication-related inquiries and empower patients to maximize their medication benefits.
  • Address complex medication-related questions from patients and healthcare professionals.
  • Facilitate medication adjustments in conjunction with the Clinical Pharmacist.
  • Conduct audits focused on patient safety in medication prescribing and share findings with the clinical team.

Job Duties

  • Engage with the practice team to provide both clinical and administrative support.
  • Work alongside GPs and the Clinical Pharmacist as needed.
  • Offer clinical and professional guidance to GPs and other primary healthcare team members.
  • Perform medication reconciliation following patient discharge and during care transitions, implementing changes as appropriate.
  • Conduct reviews to ensure compliance with monitoring and shared care protocols.
  • Provide telephonic consultations to deliver medication advice and conduct medication reviews.
  • Manage prescription inquiries within the scope of expertise.
  • Review prescription requests from nursing homes.
  • Maintain patient records, focusing on dose optimization and medication synchronization.
  • Enhance prescribing processes within the practice, which may involve training practice-based clerks to improve coordination and administration.
  • Promote electronic repeat dispensing and online ordering systems.
  • Assist clinicians in identifying alternative medications when necessary.
  • Support the practice in addressing complaints or significant incidents.
  • Collaborate with pharmacy stakeholders, including the Medicines Management Team and community pharmacy.
  • Handle sensitive and confidential information in compliance with relevant regulations.
  • Contribute to the development and review of medication audits using clinical systems.
  • Ensure timely action on major drug alerts and advise on necessary medication changes.
  • Process clinical correspondence, seeking guidance from GPs when needed.
  • Address any unlicensed medication requests with GP Partners.
  • Review clinical results within the scope of expertise.
  • Implement medication titration as directed by GPs.
  • Advocate for safe, cost-effective, and evidence-based prescribing practices.
  • Engage in ongoing training and development to maintain necessary knowledge and skills.

Required Qualifications

  • NVQ Level 2 in Pharmacy Service Skills.
  • BTEC Level 2 in Pharmaceutical Science.

Desired Qualifications

  • NVQ Level 3 in Pharmaceutical Science.
  • Professional Registration with GPhC.

Experience Requirements

  • Proven experience as a Pharmacy Technician.
  • Experience in a Primary or Secondary Care setting is advantageous.
  • Familiarity with liaising among various healthcare professionals.
  • Experience in data collection and analysis is a plus.
