About the Role

We are seeking a highly motivated and compassionate Support Coordinator to join our team at Home Group. As a Support Coordinator, you will play a vital role in supporting our customers who have complex needs to develop their skills and live more independently in the community.

Key Responsibilities

  • Develop and implement personalized support plans with customers, working collaboratively with our team of Support Workers.
  • Conduct risk assessments, support planning, goal setting, and regular reviews to ensure customers receive the best possible support.
  • Provide emotional support and guidance to customers, helping them to overcome challenges and achieve their goals.
  • Work closely with our Clinical Team to create a psychologically informed environment that promotes customer well-being.
  • Collaborate with colleagues to ensure seamless delivery of support services.
