We are seeking a highly skilled and experienced Care Coordinator to join our team at Active Care Group. As a Care Coordinator, you will play a vital role in ensuring the delivery of high-quality care services to our clients.
Key Responsibilities- Provide exceptional customer service to clients and their families, responding to their needs and concerns in a timely and professional manner.
- Coordinate and schedule care packages to ensure continuity of care for clients in their own homes.
- Work closely with the care team to ensure that all care services are delivered to the highest standards, meeting the needs of clients and their families.
- Manage and maintain accurate records of client care plans, ensuring that all relevant information is up-to-date and easily accessible.
- Collaborate with other departments, such as clinical and business leadership teams, to ensure seamless delivery of care services.
- Participate in on-call rota to provide 24/7 support to clients and their families.
- Experience as a Care Coordinator or in a similar role.
- Previous experience of using booking and phone systems.
- Ability to deal with conflict in a calm and supportive manner.
- Strong work ethic and a dynamic, flexible approach to the role.
- Excellent customer service skills and ability to communicate effectively with clients and their families.
- Ability to work as part of a team, alone, and think outside the box.
