About the Role

We are seeking a highly skilled and experienced Care Coordinator to join our team at Active Care Group. As a Care Coordinator, you will play a vital role in ensuring the delivery of high-quality care services to our clients.

Key Responsibilities
  • Provide exceptional customer service to clients and their families, responding to their needs and concerns in a timely and professional manner.
  • Coordinate and schedule care packages to ensure continuity of care for clients in their own homes.
  • Work closely with the care team to ensure that all care services are delivered to the highest standards, meeting the needs of clients and their families.
  • Manage and maintain accurate records of client care plans, ensuring that all relevant information is up-to-date and easily accessible.
  • Collaborate with other departments, such as clinical and business leadership teams, to ensure seamless delivery of care services.
  • Participate in on-call rota to provide 24/7 support to clients and their families.
Requirements
  • Experience as a Care Coordinator or in a similar role.
  • Previous experience of using booking and phone systems.
  • Ability to deal with conflict in a calm and supportive manner.
  • Strong work ethic and a dynamic, flexible approach to the role.
  • Excellent customer service skills and ability to communicate effectively with clients and their families.
  • Ability to work as part of a team, alone, and think outside the box.
  • Excellent communication and interpersonal skills.
  • Willingness to learn and develop.
