Job Title: Staff Nurse - Palliative Care Specialist

We are seeking a highly skilled and compassionate Staff Nurse to join our Patient and Carer Wellbeing Service team at The Myton Hospices. As a Palliative Care Specialist, you will play a vital role in providing high-quality care and support to patients and their families living with life-limiting illnesses.

Key Responsibilities:
  • Coordinate the assessment, planning, and evaluation of individualized patient-centred care, with a holistic approach that is sensitive and responsive to the ever-changing needs of patients, families, and carers.
  • Work closely with a multidisciplinary team to provide compassionate, high-quality, and evidence-based care within the service, maximizing the specialist palliative care and end-of-life skills of staff.
  • Engage and positively deliver individual and group activities of support and education aligned to the service, for patients and their families, regarding the various aspects of their health and wellbeing.
  • Provide effective communication and work in partnership with patients, carers, PCWBS nursing team, and other members of the multidisciplinary team to ensure that care and support reflects individual needs.
Requirements:
  • Significant post-registration experience in palliative and end-of-life care.
  • Experience of needs assessment/care planning, multidisciplinary team working, and caring for patients and families with life-limiting/life-threatening conditions.
  • Good knowledge of assessment and symptom management.
  • Excellent personal and communication skills, teaching and assessing skills, and organizational ability.
  • Motivated and enthusiastic, with a commitment to the Myton values.
