About the Role

Aberdeenshire Council is seeking a skilled Care Manager to join our team in providing high-quality support services to older adults and individuals with physical disabilities.

Key Responsibilities

  • Undertake comprehensive assessments to identify the needs of service users and develop personalized support plans.
  • Work collaboratively with multidisciplinary teams to ensure seamless service delivery and achieve optimal outcomes.
  • Develop and maintain effective relationships with service users, their families, and carers to ensure their needs are met and their rights are respected.
  • Contribute to the development and implementation of policies and procedures that promote quality service delivery and continuous improvement.
  • Participate in training and professional development opportunities to enhance skills and knowledge in care management and social work.

Requirements

  • Relevant qualifications in social work or a related field.
  • Experience in care management, social work, or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively as part of a team.
  • Commitment to continuous learning and professional development.

Working Arrangements

This is a full-time position working 2.5 days per week.

Regulatory Requirements

This post is regulated work with children and/or protected adults under the Protection of Vulnerable Groups (Scotland) Act. The preferred candidate will be required to join the PVG Scheme or undergo a PVG Scheme update check.
