About the Role

We are seeking a compassionate and dedicated Support Practitioner to join our team at Cornerstone Community Care. As a Support Practitioner, you will play a vital role in providing high-quality care and support to individuals with complex learning and physical disabilities.

Key Responsibilities
  • Provide personal care and support to individuals with complex needs, including assistance with daily living activities, personal care, and medication management.
  • Support individuals with their hobbies and interests, including music, films, in-house activities, and community outings.
  • Assist with household tasks, such as cooking, cleaning, and laundry.
  • Develop and maintain positive relationships with individuals, their families, and other stakeholders.
  • Work collaboratively with the team to ensure a high-quality service is delivered.
Requirements
  • A caring and compassionate attitude, with a commitment to providing person-centered care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and colleagues.
  • Good organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • A positive attitude, with a willingness to learn and develop new skills.
  • Confidence in the use of computers and other technology.
