We are seeking a compassionate and dedicated Support Practitioner to join our team at Cornerstone Community Care. As a Support Practitioner, you will play a vital role in providing high-quality care and support to individuals with complex learning and physical disabilities.
Key Responsibilities- Provide personal care and support to individuals with complex needs, including assistance with daily living activities, personal care, and medication management.
- Support individuals with their hobbies and interests, including music, films, in-house activities, and community outings.
- Assist with household tasks, such as cooking, cleaning, and laundry.
- Develop and maintain positive relationships with individuals, their families, and other stakeholders.
- Work collaboratively with the team to ensure a high-quality service is delivered.
