We are seeking a skilled Resettlement Worker to join our team in Chester on a temporary basis with potential for permanency. This is a 35 hour per week role, working across 3 days (Thursday, Friday and Sunday) on a set shift pattern of 7am to 8pm.

As a Resettlement Worker, you will be supporting clients with complex needs who are living in supported accommodation to regain independence and sustain a tenancy when moving on from the service.

Your Key Responsibilities

  • Conduct needs and risk assessments, and co-create support plans with the customer to achieve positive, achievable and motivating outcomes.
  • Provide practical help and support with budgeting, life skills, healthcare, nutrition and welfare benefits and housing applications.
  • Ensure quick turnarounds of void properties to maximise income.
  • Manage low-level anti-social and challenging behaviours.
  • Signpost customers to external agencies and support services for mental health, debt and substance/alcohol misuse.
  • Encourage customers to access and get involved with external services within the community to improve wellbeing and employment prospects.
  • Attend relevant multi-agency meetings and work in partnership with housing providers, community groups and charities and Local Authority.
  • Carry out routine inspections and room checks, routine fire safety checks and report repairs.
