About the Role

Oxleas NHS Foundation Trust is seeking a skilled and compassionate Continuing Care Assistant to join our Community Children's Nursing team. As a key member of our team, you will provide high-quality, specialist care to children and young people with complex health needs in their own homes and community settings.

Key Responsibilities

  • Work as part of a multidisciplinary team to deliver individualized packages of care to children with complex health needs.
  • Provide skilled clinical care, including tracheostomy care, enteral feeding, and basic life support, under the supervision of a Lead Continuing Care nurse.
  • Carry out safety checks of technical equipment and report any issues to the family and Assistant Practitioner.
  • Identify concerns and report to the continuing care nurse, implementing agreed individual programs of care as required.
  • Work as part of a team to provide support and advice to families within your scope of practice.

Person Specification

  • NVQ 3 or the Cavendish Care certificate.
  • Experience working with children with disabilities and complex health needs.
  • Ability to work as part of a team and provide high-quality care in a fast-paced environment.
