Job Summary

We are seeking a highly skilled and dedicated Children's Continuing Care Associate Practitioner to join our team at Oxleas NHS Foundation Trust. As a key member of our Greenwich and Bexley Special Children's Continuing Care nursing team, you will play a vital role in providing high-quality care to children with complex healthcare needs.

Key Responsibilities
  1. Care Planning and Delivery: Collaborate with the Children's Community Nursing Team to develop and implement individualized care plans for children with complex needs.
  2. Clinical Skills: Perform skilled clinic tasks, including enteral feeding, suctioning, and catheterization, as well as basic life support and safety checks.
  3. Documentation and Record-Keeping: Maintain accurate and timely written electronic records in accordance with Trust and service policies.
  4. Equipment Management: Familiarize yourself with equipment required to support children and report any deficiencies to the team.
  5. Observation and Reporting: Independently interpret observations, recognize abnormal indicators, and report to a registered nurse as required.
  6. Emergency Situations: Identify and respond to emergency situations, taking appropriate actions to ensure the safety and well-being of children.
  7. Medication Administration: Follow Trust policy and administer routine medications as trained.
