About the Role

We are seeking a highly skilled and compassionate Care Coordinator to join our team at PeoplePlus. As a Care Coordinator, you will play a vital role in supporting individuals with disabilities to live independently in their local communities.

Key Responsibilities

  • Provide person-centred support to individuals, enabling them to focus on achieving their desired outcomes.
  • Manage a caseload within a designated geographic area, including home visits and community engagement.
  • Assist individuals in managing their care and support plans, ensuring they meet their needs in a cost-effective and safe manner.
  • Collate and maintain accurate records of all communication and caseload management.
  • Ensure all information provided is clear, concise, and accessible to individuals with varying communication needs.
  • Organise and attend meetings and reviews with individuals, colleagues, and other professionals as required.

Requirements

  • Background in social care, with experience supporting individuals with disabilities.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong organisational and time management skills.
  • Proficiency in using databases and technology to manage caseloads and records.
