As a key member of the team, you will be responsible for providing high-quality nursing care to patients in their own homes or chosen place of residence.
Main Responsibilities- Conduct holistic assessments of patients' care needs and develop individualized care plans.
- Provide nursing care to patients with complex and simple care needs, including delegation of care to other team members as appropriate.
- Work collaboratively with other healthcare professionals to promote independence and timely discharge for patients.
- Participate in the development and implementation of care pathways and protocols to improve patient outcomes.

Requirements- RGN NMC Registered
- Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes
- Post registration general experience
- Experience of working in a multi-disciplinary/multi-agency health/social care environment
- Experience and knowledge of basic administration systems, including Microsoft Office
- Experience of successful partnership working arrangements with other services
- Previous community experience
