Job Summary
We are seeking a skilled and compassionate Community Health Specialist to join our dedicated team at Wiltshire Health and Care LLP. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to patients in their own homes and residential settings.
Main Responsibilities
As a Community Health Specialist, you will be responsible for:
- Providing comprehensive care to patients in their own homes and residential settings
- Collaborating with General Practitioners and community services to ensure seamless care delivery
- Participating in initiatives to prevent unnecessary hospital admissions and promote patient independence
- Conducting assessments and care planning to meet individual patient needs
Requirements
To be successful in this role, you will need:
- Access to a vehicle for business purposes and a valid driving license
- Registered Nurse certification
Desirable Qualifications and Experience
We welcome applications from candidates with:
- Teaching and Assessing in Clinical Practice
- Additional relevant qualifications
- Non-medical prescribing course
- Awareness of recent Government Papers influencing clinical practice
