Job Summary:

We are seeking a highly skilled and dedicated Social Worker to join our NHS Continuing Healthcare Team at Hampshire County Council. As a qualified Social Worker, you will work collaboratively with Health colleagues to support individuals with their onward care journey following a Discharge to Assess period from hospital.

Key Responsibilities:

  • Undertake assessments under the Care Act 2014 and for NHS CHC to work in partnership with individuals and their families to move them to appropriate care provision following assessment.
  • Work collaboratively with colleagues to achieve successful outcomes within a dynamic multi-disciplinary environment.
  • Develop and implement care plans that meet the needs of individuals, ensuring their safety and well-being.
  • Provide expert advice and guidance to colleagues and stakeholders on adult care and support.

Requirements:

  • UK based Adults' Social Work experience with relevant professional registration.
  • Strong analytical and problem-solving skills, with the ability to work in a fast-paced environment.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals, families, and colleagues.
  • A commitment to delivering high-quality care and support, with a focus on person-centered practice.

Travel Requirements:

You will be required to travel to various locations within Hampshire and it is essential that you have a current driver's license and access to a reliable vehicle on a regular basis.
