Overview of the Role

As a Recuperation and Support Facilitator, you will play a vital role in providing assistance and time to a designated group of individuals, thereby fostering their recovery and helping them sustain their autonomy within the community.

This position encompasses various tiers of facilitators: Foundation, Intermediate, and Senior. These tiers are determined by (i) levels of expertise and training, (ii) the complexity of cases handled and the degree of oversight provided to fellow facilitators, and (iii) the extent of independence and initiative required for the role. In the Intermediate tier, you will engage with individuals facing intricate challenges with minimal supervision, while also providing guidance and support to other facilitators.

Key Responsibilities

The Recuperation and Support Facilitators are integral members of a team dedicated to mental health services, focusing on the daily holistic support requirements of individuals. This may involve collaboration across different organizations and teams. Your work will be coordinated through the Care Programme Approach (CPA), assisting care coordinators in assessing, planning, implementing, and evaluating personalized care strategies.

Primary Duties

  • To mentor and collaborate with Foundation Facilitators as part of their overall support and development program.
  • To actively promote the recovery and independent living of individuals within the community.
  • To assist individuals in effectively engaging with their personalized care plans and facilitate feedback to their care coordinators.
  • To liaise with and provide information to other agencies involved in supporting the individual.
  • To adhere to recovery principles, enabling individuals to regain control, responsibility, hope, and enjoyment.
  • To cultivate meaningful relationships with individuals while maintaining appropriate and transparent boundaries, seeking support and supervision as needed.
  • To provide consistent and practical support to individuals and their caregivers in fostering independence and preserving dignity and self-respect, including assistance with daily living tasks.
  • To support individuals in addressing their holistic needs (education, welfare, financial, leisure, housing, spiritual, etc.) in collaboration with other stakeholders.
  • To advocate for physical, emotional, mental, and spiritual well-being.
  • To assist individuals in recognizing early signs of relapse and accessing timely support from various resources.
  • To maintain accurate records in accordance with existing policies, documenting relevant details in case notes as necessary.
  • To participate in staff meetings, training sessions, and supervision.
  • To complete the Vocational Related Qualification (VRQ) level 2 Certificate in Mental Health Work and the National Vocational Qualification (NVQ) in Social Care and Health level 3.
  • To contribute to service development as an equal partner alongside individuals and caregivers.
  • To undertake any additional duties as may be assigned within the general scope of the position.
