About the Role

We are seeking a highly skilled and compassionate Mental Health Specialist to join our team at Somerset NHS Foundation Trust. As an Early Intervention Practitioner, you will play a vital role in providing high-quality care and support to individuals experiencing their first episode of psychosis.

Key Responsibilities
  • Provide assessments, care coordination, and outreach services to individuals and their families.
  • Develop and implement individualized care plans to reduce the duration of untreated psychosis.
  • Work collaboratively with multidisciplinary teams to provide comprehensive care and support.
  • Deliver individual therapy and family interventions to promote recovery and well-being.
  • Participate in training and professional development opportunities to enhance skills and knowledge.
Requirements
  • Registered Mental Health Nurse, Social Worker, or Occupational Therapist qualification.
  • Current registration with relevant professional body.
  • Experience in multi-disciplinary community mental health services.
  • Good knowledge of resources and working practices across health and social care.
  • Ability to work autonomously and as part of a team.
  • Excellent communication and interpersonal skills.
