Job Summary

The Northamptonshire Community Stroke Team is seeking a skilled Rehabilitation Assistant to join our team. As a key member of our multidisciplinary team, you will work closely with patients, families, and healthcare professionals to provide high-quality rehabilitation services.

Main Responsibilities
  • Provide goal-based reablement packages of care to individuals post-stroke, promoting independence and optimal recovery.
  • Assess and make recommendations for equipment, home adaptations, and ongoing rehabilitation, reporting to therapists, nurses, or care managers.
  • Deliver, fit, and demonstrate basic equipment to clients and relevant others, ensuring client safety.
  • Provide advice to patients, families, and long-term carers on the safe use of equipment and techniques to optimize mobility, functional ability, safety, and independence.
  • Implement and record planned programs of care and rehabilitation, including the use of paper notes, electronic notes, and virtual platforms.
  • Support patients through instruction and guidance in therapeutic activities, under the direction of a therapist, to achieve identified treatment goals.
  • Involve clients and carers in the planning of care/rehabilitation programs and encourage self-management where appropriate.
Requirements
  • Good level of basic education (GCSE or equivalent) in English and Maths.
  • Study to NVQ level 3 or equivalent experience.
  • Experience working face-to-face with members of the public.
  • Experience working as a member of a team.
  • Recent NHS experience.
  • Experience working in a community health or social care setting.
  • Experience of working with individuals with stroke.
  • Basic knowledge and understanding of Stroke.
  • Basic knowledge and understanding of the role of Occupational Therapists, Speech and Language Therapists, and Physiotherapists in healthcare.
  • Awareness of risks to Health & Safety in the community.
Desirable Qualifications and Experience
  • Experience working as a Band 2 or 3 clinical support worker within Therapies and/or stroke.
  • Awareness of the impact of stroke on an individual.
  • Good experience of working with community healthcare teams.

  • Non-judgmental and able to work with people from a variety of backgrounds.
  • Physical ability to undertake manual handling of patients and equipment on a daily basis in a variety of environments.
  • Willing to work to meet the needs of the service (to include evenings, weekends, bank holidays).
  • Be a team player and support colleagues as required.
