About the Role

We are seeking a highly motivated and compassionate Rehabilitation Assistant to join our Community Stroke Team at Northampton General Hospital. As a key member of our team, you will play a vital role in providing rehabilitation and care to patients in their own homes or place of residence.

Key Responsibilities
  • Provide goal-based reablement packages of care to individuals post-stroke, promoting independence and mobility.
  • Follow rehabilitation plans as directed by therapists or nurses, ensuring patients receive high-quality care and support.
  • Assess and make recommendations for equipment, home adaptations, and ongoing rehabilitation, reporting to therapists, registered nurses, or care managers.
  • Deliver, fit, and demonstrate basic equipment to clients and others, ensuring client safety.
  • Provide advice to patients, their families, and long-term carers, teaching and demonstrating the safe use of equipment or techniques to optimize patients' mobility, functional ability, safety, and independence.
  • Implement and record planned programs of care and rehabilitation, using paper notes, electronic notes, and other communication methods.
  • Support patients through instruction and guidance in therapeutic activities, under the direction of therapists, to achieve identified treatment goals.
  • Involve clients and carers in the planning of care/rehabilitation programs and encourage self-management where appropriate.
  • Build professional relationships with patients, their families, and carers, using effective communication, reasoning, and negotiating skills, respecting others' views, culture, autonomy, and individuality.
Requirements
  • Good level of basic education (GCSE or equivalent) in English and Maths.
  • Study to NVQ level 3 or equivalent experience.
  • Experience working face-to-face with members of the public.
  • Experience working as a member of a team.
  • Recent NHS experience.
  • Experience working in a community health or social care setting.
  • Experience of working with individuals with stroke.
  • Basic knowledge and understanding of stroke.
  • Basic knowledge and understanding of the role of occupational therapists, speech and language therapists, and physiotherapists in healthcare.
  • Awareness of risks to health and safety in the community.
Desirable Qualifications and Experience
  • Experience working as a Band 2 or 3 clinical support worker within therapies and/or stroke.
  • Awareness of the impact of stroke on an individual.
  • Good experience of working with community healthcare teams.

  • Non-judgmental and able to work with people from a variety of backgrounds.
  • Physical ability to undertake manual handling of patients and equipment on a daily basis in a variety of environments.
  • Willing to work to meet the needs of the service (including evenings, weekends, and bank holidays).
  • Team player and able to support colleagues as required.
