We are seeking a highly skilled and compassionate Wellbeing Practitioner to join our dynamic team at Cornwall Partnership NHS Foundation Trust. As a key member of our Home Treatment Team, Mental Health Connect line, and Crisis Hub, you will play a vital role in providing high-quality care and support to individuals experiencing mental health challenges.

Key Responsibilities
  • Work collaboratively with a multidisciplinary team to provide interventions, support, guidance, and signposting to individuals in crisis.
  • Rotate between working in Home Treatment Team, the Crisis Hub, and the Mental Health Connect Helpline to broaden your experience and add variety to the post.
  • Develop and maintain strong relationships with service users, their families, and carers to ensure comprehensive and person-centered care.
  • Contribute to the development and implementation of innovative and effective care pathways to improve outcomes for individuals with mental health needs.
