Job Summary

We are seeking a skilled Health and Wellbeing Support Worker to join our dynamic team at North Kerrier East Primary Care Network (PCN). As a key member of our team, you will work closely with clinicians, mental health practitioners, and care coordinators to support adults and children across our local population.

About the Role

The Health and Wellbeing Support Worker will manage and prioritize their own caseload, helping individuals take proactive steps to improve their physical and mental health conditions. You will use coaching skills, motivational interviewing, and a holistic approach to identify goals for care and activation, with a possible case management approach.

You will also develop and contribute to multi-disciplinary team meetings across the PCN and with partners, effectively managing cases with complexity. Additionally, you will help and support people to access self-management education courses, peer support, or other interventions that promote health and wellbeing.

Key Responsibilities
  • Manage and prioritize your own caseload to support individuals in improving their physical and mental health conditions.
  • Use coaching skills, motivational interviewing, and a holistic approach to identify goals for care and activation.
  • Develop and contribute to multi-disciplinary team meetings across the PCN and with partners.
  • Help and support people to access self-management education courses, peer support, or other interventions that promote health and wellbeing.
