About the Role

We are seeking a highly skilled and compassionate Social Prescribing Link Worker to join our team at Primary Healthcare Darlington. As a key member of our Wellbeing Service, you will play a vital role in empowering individuals to take control of their health and wellbeing.

Key Responsibilities
  • Develop and maintain strong relationships with local agencies, GP practices, and community groups to promote the Wellbeing Service and facilitate referrals.
  • Conduct comprehensive wellbeing assessments, taking into account the impact of wider determinants of health on individuals' lives.
  • Co-produce personalized wellbeing plans with individuals, connecting them to community groups, activities, and statutory services that meet their needs.
  • Provide personalized support to individuals, their families, and carers, focusing on building trust, promoting independence, and improving health outcomes.
  • Work collaboratively with local partners to ensure community groups are sustainable, and community assets are nurtured.
  • Collect and analyze data to track the impact of social prescribing on individuals' health and wellbeing.
Requirements
  • GCSE/NVQ level 2/3 or equivalent level of training and experience.
  • Understanding of basic health and social care terminology.
  • Proven ability to recognize and manage risk, knowledge of the needs of older people, safeguarding, and the associated legislative framework.
  • Good interpersonal skills, ability to communicate confidently with staff at all levels.
  • Ability to work well across teams, good time management, and an ability to prioritize and work to strict deadlines.
  • Ability to work on own initiative without direct supervision, understanding where clinical input is needed.
  • IT skills and experience in the use of Microsoft Office programmes.
Desirable Qualifications and Experience
  • Familiarity with IT systems currently in use in health and social care settings.
  • Experience in working directly in a community development context, adult health and social care, learning support, or public health/health improvement.
  • Experience in supporting people, their families, and carers in a related role.
  • Experience in supporting people with their mental health, either in a paid, unpaid, or informal capacity.
  • Experience in working with the VCSE sector, including with volunteers and small community groups.
