About the Role

We are seeking an enthusiastic and experienced nurse, social worker, OT, or other suitably registered person to work as a Band 6 Specialist Intensive Practitioner in our Swindon Intensive Service.

Key Responsibilities
  • Lead shifts as a shift coordinator, reviewing and accepting referrals, completing thorough core assessments, risk management, and care plans.
  • Implement risk management and care plans for the effective home treatment of service users, utilizing least restrictive practice at every opportunity.
  • Manage medication and concordance within the community.
Requirements
  • Diploma level/degree in a relevant health/social care profession.
  • Substantial post-registration experience in frontline adult mental health services, undertaking assessment and delivering intervention.
  • Developed understanding of recovery principles and the role of secondary mental health services in delivering care.
